Default Lost Medals and how to get them replaced.

Typical Question here:

All my medals were lost when I moved. How can I get them replaced?

Replacement medals are not provided by the U.S. Department of Veterans Affairs (VA).

They are available from the National Personnel Records Center (NPRC), which is under the jurisdiction of the National Archives and Records Administration.

To request replacement medals, complete Standard Form 180, Request Pertaining to Military Records.

You can request the form or Download a copy via the above link.

Fill in as much information as possible and send it to the correct address listed on the form. You must also include a copy of the veteran’s military discharge papers (DD Form 214) stating the awarded medals in order to receive replacements.

If you do not have the DD Form 214 or equivalent, you need to request that before you request the medals and ribbons.
