Request for lost DD214

How can I get a copy of my DD Form 214?

The DD Form 214, Report of Separation, is filed in the Official Military Personnel File. Certain basic information is needed to locate military service records. This information includes the veteran's complete name used while in service, service number or social security number, branch of service, and dates of service.

You need to obtain a Standard Form 180 and fill this out to mail into the National Personnel Records Center.

This form will need to be completed and mailed to:

Costs.

Generally there is no charge for military personnel and health record information provided to veterans, next-of-kin, and authorized representatives. If your request involves a service fee, you will be notified as soon as that determination is made.

Response Time.

Response time varies dependent upon the complexity of your request, the availability of records, and our workload. Please do not send a follow-up request before 90 days have elapsed as it may cause further delays.

One place to check first is with the VA Regional office in your area. They should have a copy of your DD 214.

Actually, they don't physically 'have it', but they can generally get it more quickly than if you request it directly. I've done it both ways, and going through the VA took less than half the time.

That depends on when you got out. As a general rule, they won't have a copy if you didn't:

1- Retire with a disibility

2- request they get one

but all of this may be out the window with the new Army... Your best bet would have been to have a copy noterized at you local courthouse when you exited the military. That way they would be able to provide you with a noterized copy when you needed one...

Interesting tidbit.

Some how I misplaced my last DD-214 when I applied at my local Veterans Outreach Center for help locating a new Job.

I followed my own advice here and just received back a letter stating that because I retired from the Air Guard, That the Adjutant General's Office of my state would have this on file.

I'll have to check this out and report back to everyone with a new update.

I too am missing my dd 214. I was a reservist and mobilized to approx. 300 days of active duty in 2003. I received a dd 214 at the end of our mobilization, but somehow misplaced it. Now the national archives service has no record of my active duty service. Any idea how I can get it replaced or recreated? I'm still in contact with members from my unit, including my 1SG who's still with them.

Might check IPERMS, I saw a notice saying any one enlisted after 2001, still has access to IPERMS, might check there,

If you still have access to AKO, log in and click on the link halfway down the home page title OMPF. Anyone that has been in the Army (probably other branches too, I only know about the Army for sure) in recent years has a lot of their official Military paperwork electronically filed. I just pulled mine up and both of my DD214's are on there under the Documents tab.
